How Long Should a Coaching Engagement Last?

I hear this from nearly every physician who reaches out for the first time. "How long does coaching take?" It is a fair question. You are busy. You want to know the coaching engagement length before you commit your calendar and your budget.
Here is my honest answer: most coaching engagements in academic medicine run three to six months. That is not a guess. Research from the International Coaching Federation consistently shows that executive coaching engagements averaging three to six months produce the strongest outcomes. And that tracks with what I see in my own practice.
Why Three to Six Months Works
Think of it like a training camp. You need enough time to identify what you are working on, practice new approaches in real situations, and build enough repetition that the changes stick. Less than three months rarely gives you that full cycle.
A typical engagement looks like this:
- Month one: Clarify the real issue. What you come in with is not always the thing that matters most.
- Months two and three: Work the issue. Try new behaviors. Get feedback. Adjust.
- Months four through six (if needed): Reinforce the changes. Handle the complications that show up once you start doing things differently.
In The Coaching Habit, Michael Bungay Stanier describes coaching as helping people "work less hard and have more impact." That does not happen in a single conversation. It happens over a series of them.
When Longer Makes Sense
Some situations call for more time. If you are stepping into a major leadership role for the first time, six months may not be enough. If you are navigating a career transition with high uncertainty, the timeline stretches. And if the first round of work uncovers a deeper question you did not know you had, extending is a sign of progress, not stalling.
How to Know When You Are Done
The clearest signal is this: you are already doing the thing we would have talked about. You had the hard conversation. You set the boundary. You made the decision without needing to process it with me first.
When that starts happening consistently, you are done. A good coach will tell you so. If yours does not, trust yourself to name it.
Set a Starting Point, Not a Finish Line
Commit to three months. Reassess at the end. That gives you enough runway to do real work without locking yourself into a timeline that does not fit.
